IMPORTANT POINTS :
1. In pursuance to advertisement notification No: 18296-303 dated 30-12-2021, it is hereby notified that the list of District wise provisionally selected BAMS/ Bums BSc Nursing/GNM Candidates for undergoing six (6) months Certificate Programme in Community Health (CPCH) through IGNOU or University of Kashmir and Direct Placement as MLHP/CHO (in case of BSc Integrated Candidates) for session January 2022, as Annexure “A”
2 All provisional selected candidates shall bring original documents (D.O.B, Domicile Certificate, Degree Certificate & Registration Certificate) for verification.
In case of Candidates, who claim to possess the Integrated B.Sc Nursing degree shall also produce valid authentication certificate/Degree issued by the concerned recognized university only for direct placement as MLHP/CHO.
3. All Provisionally selected candidates shall submit a Fixed Deposit Receipt (FDR) of Rs 60000/- for one (1) year in favour of Financial Advisor & Chief Accounts Officer (FA&CAO) NHM, J&K which shall be released after completion of 6 months in- service period as MLHP/CHO at a designated HWC- SC (BAMS/ BUMS/ BSc Nursing/GNM Candidates).
However, in case of the Integrated BSc Nursing candidates, Fixed Deposit Receipt (FDR) shall be released after completion of I year in-service period as MLHP/CHO at a designated HWC- SC, duly certified by concerned Block Medical Officer and the FDR shall be forfeited in case selected candidate fails to join or leaves the job before completion of stipulated time period.
4. All Provisionally selected candidates for undergoing six (6) months Certificate Programme in Community Health (CPCH) for January 2022 session through IGNOU and University of Kashmir and Direct Placement as MLHP/CHO(in case of BSc Integrated Candidates) shall submit an affidavit issued by 1st Class Judicial Magistrate (as per respective Performa available on official website www.jknhm.com)
Verification of original documents, submission of FDR & Affidavit shall be held as per following schedule:
NOTE:
Any candidate who is already working as Regular/ NHM Contractual Govt. employee is not allowed to undergo the said course.
Further, if found that any of the candidates has concealed any material fact or has been selected on the basis of any misrepresentation during the process of selection, he / she shall be debarred from undergoing the six (6) months Certificate Programme in
Community Health (CPCH), besides he/ she shall be liable for legal action.
In case any selected candidate fails to join from a district, waiting list shall be
formulated for such district only.
